Improving Diabetes Care and Outcomes on the South Side of Chicago

Get Involved, Participate!

We’d love to have you participate in our project. Here’s how:

Come to one of our events.

We’d love to meet you! Check out our upcoming events by clicking here.

Sign up for the Diabetes Empowerment Program

The Diabetes Empowerment Program is designed to help people with diabetes better understand and manage their disease. In a group setting, there are lively discussions and hands-on activities to learn skills such as taking your blood sugar and tips on how to best communicate with the doctor to help get the care you need. The class is also a chance to learn about the diabetes resources in the community, through events like grocery store tours or local exercise programs!

We don’t have room for everyone right away, but if you tell us you are interested, we’ll contact you as soon as a new group starts. There may be a small charge for participating, although some groups are free.

To sign up to be on our group list, email

Volunteer at a Community Event

We participate in a variety of community events, and we often need extra hands to help us!

Volunteers may help with health information tables, cooking demonstrations, or event production. No experience necessary!
